Family Options is committed to providing services to adults with disabilities. We approach each consumer with an individualized plan of care that promotes positive growth and empowerment to the individuals we serve. The heart of Family Options’ services are relationships that are nurtured and developed among a “Circle of Support”. Family Options’ builds these relationships on respect, connections, and commitment to improving quality of life to the individuals we serve.
Adult Family Home (FHA)
- This program offers adults with developmental disabilities a family home environment also known as Adult Foster Family Program.
- Individuals have an opportunity to live in a less restrictive environment and is an alternative to institutional care, hospitals or group homes.
- Individuals will have their own bedroom within the family home.
- Professional Services: Psychiatry, Counselor, Behavioral Management, Nutritional, Speech Therapy, Nursing, Group Therapy and others as needed.
- Respite Services.
- Individual and group social activities.
Supportive Living Services (SLS)
- Individuals/Consumers/Clients live in their own apartments/homes and receive training and assistance from Direct Support Professionals 24/7.
- Individuals have a circle of support which works together as a team to assist in creating individualized service plans.
- Individual services, training and supports are tailored and flexible in order to assist developing and achieving realistic goals.
Independent Living Program (ILP)
- Individuals/Consumers/Clients live in their own apartments/homes and receive training and assistance from Direct Support Professionals on an hourly basis.
- Individuals have a circle of support which works together as a team to assist in creating individualized service plans and schedule for services.
- Individual services, training and supports are tailored and flexible in order to assist developing and achieving realistic goals.
